    Is there a proper way to ask for a review? (email? verbal request?)

    I would suggest making an appointment with your manager (or even just asking casually) to talk about how the company typically administers job reviews. They may or may not do it at 6 months and I think it would sound like more of a conversation to better yourself if you asked it like that.

    It depends what your company policy is. Most do an introductory period eval at 3mos and then annually thereafter. Check with your HR dept or look it up in your manual to see what your company's policy actually is.

    Also, while most employers are to evaluate staff annually, if their HR Dept/ Sr. Manager do not enforce... most likely the eval process is falling by the waste side. However, any employee has the right to request an evaluation in accordance with the company policy.
