i havent needed it ever but I didn't think you could get instant titles from NYS DMV, pretty sure this is a come in the mail thing, might be able to pay extra to expedite it.

    http://www.dmv.ny.gov/title/tdpfaq.htm

The DMV will normally send a title certificate by US Postal Service first class mail. If you order a duplicate title certificate online, you can order express mail delivery for an additional fee.

How long will it take to receive my duplicate title certificate?

Your duplicate title certificate will be sent to you by mail or an express delivery service. If you choose delivery by regular First Class Mail, allow five to seven business days for delivery of your duplicate title certificate.
If you select an express delivery service, you must be at the delivery address on the delivery date, up to the delivery time, and you must provide your signature to receive the document. If you select an express delivery service, make sure you note the exact date and time that your title will be delivered.
An express delivery carrier can have special requirements for document delivery. The requirements can include that the person who receives the document and signs for it be age 21 or older. Contact the express delivery carrier you select about their requirements.
If you order a title certificate online before 8 p.m. Eastern Time, your title certificate will be processed on the first business day after your order. If you order a title certificate online after 8 p.m. Eastern Time, your title certificate will be processed two business days after your order.
